MyChart: Appendix A – Instructions for Use

The red text next to each treatment indicates whether or not you have currently been taking that treatment for that condition, and how effective it has been so far (1 meaning very effective and 5 meaning ineffective). You can also see a list of all of your medications, supplements, and other medical devices that are related to any of your health conditions (if applicable).

To add a new condition or treatment to MyChart, click on the “+ Add…” button located below the list of conditions and treatments. You will be prompted to enter some basic information about your health condition, such as its name and description. After you’ve added a new condition or treatment, it will appear in the list of conditions and treatments on the MyChart website.

To edit information about a condition or treatment, click on the “Edit…” button located below the list of conditions and treatments.
